Perg. 120, fol. 185v-187v or Darmstadt, Universitäts- und Landesbibliothek, Hs. 1215, three fragmentary flyleaves (for the latter, see Konrad Goehl, Johannes Gottfried Mayer, and Kurt Hans Staub, "Das Darmstädter Johannes-Alexandrinus-Fragment", Sudhoffs Archiv, 84 [2000], pp. 201-221.
