The parchment is heavily worn, and both the text and the musical notation are difficult to read.

Rubricated lombard (2 lines).Because of the heavily worn condition of the notation, the precise form of the punctum cannot be established with certainty. The fragment may therefore preserve either Messine Gothic notation or German (Hufnagel) notation, the latter being more likely if the isolated punctum originally had the characteristic horseshoe-nail form. The neumes are written on four black staff lines. No clefs are visible in the surviving fragment, although their presence in the original codex cannot be excluded. One custos is visible in the turn-ins.Since Petrus Ramus’s work in the host volume was adapted by the Transylvanian scholar Georgius T. Martonfalvi, and the earliest attested owner is certain Miklós Szatmárnémeti (Co. Satu Mare), a Transylvanian origin for the manuscript fragment may also be considered. This hypothesis would be further strengthened if the musical notation could be identified as Messine Gothic.Former owners: "Est Michaelis S. Annotations:
1. At the top of the page, there is an ownership inscription, but the names cannot be read clearly. Further down, the page bears partially legible Latin scholastic annotations concerning categorial beings, beings of reason, essence, and existence. A possible personal name and the probable date 1748 appear in the lower right-hand corner, together with a later collection or shelf mark.
The page contains several handwritten Latin annotations originating from an educational, probably ecclesiastical or collegiate, context. At the top appears the moral maxim "Memento mori, et in aeternum non peccabis" ("Remember that you must die, and you will not sin for ever"). Below it is the traditional rhetorical and logical formula “Quis, quid, ubi, quibus auxiliis, cur, quomodo, quando,” followed by explanations identifying the agent, object, place, means, purpose, manner, and time of an action. A partially legible philosophical note at the bottom concerns the formal nature of universals.
An inscription originating from a school or ecclesiastical educational setting can be seen on the back cover; it contains a prayer for the students and their superiors: "Precat amor mentis, nitidi et sapientiae, [?] Peccatori mortuus [est enim?] Tremendum semper nobis [?] clava Herculis, miserrimis [?] Pro incremento studiorum et Ad Christum. Gen. Parenti Scholasticum. Qui pueros ad teneros pie Christe vocasti: Excipiens gremio, Ad te confugimus precibus, nos respice vita Ut meare mos.
